STIIIZY data breach settlement: cash payment plus credit monitoring, no proof needed

What this settlement is about
In re STIIIZY, Inc. Data Breach Security Litigation (C.D. Cal., No. 2:25-cv-00490) covers an October 2024 incident in which an unauthorized party accessed customer records held by the retailer's systems — including personal information and government-issued IDs. STIIIZY denies wrongdoing. The fund is non-reversionary: whatever isn't claimed is donated, not returned.
Who qualifies and how much
Anyone whose information was accessed or stolen in the incident — in practice, anyone who got the email or postcard notice, though the form also accepts a certification that you have reason to believe you were affected. Two mutually exclusive cash options: a pro-rata share with zero paperwork (doubled if you were a California resident when you did business with STIIIZY), or documented out-of-pocket losses up to $7,500 incurred on or after October 10, 2024. A denied documented-loss claim automatically falls back to the pro-rata payment, and everyone can add the free credit monitoring either way.
How to file
The claim form asks for the Class Member ID from your notice; the administrator retrieves it at 1-855-907-2590 if you can't find it.
